Nutrition, Anabolism, and the Wound Healing Process: An Overview

Abstract:

Objective:
To develop a clear, concise, and up-to-date treatise on the role of anabolism from nutrition in wound healing. Special emphasis was to be placed on the effect of the stress response to wounding and its effect.
Methods:
A compilation of both the most important and most recent reports in the literature was used to also develop the review. The review was divided into sections to emphasize specific nutrition concepts of importance.
Results:
General and specific concepts were developed from this material. Topics included body composition and lean body mass, principles of macronutritional utilization, the stress response to wounding, nutritional assessment, nutritional support, and use of anabolic agents.
Conclusions: We found that nutrition is a critical component in all the wound healing processes. The stress response to injury and any preexistent protein-energy malnutrition will alter this response, impeding healing and leading to potential severe morbidity. A decrease in lean body mass is of particular concern as this component is responsible for all protein synthesis necessary for healing. Nutritional assessment and support needs to be well orchestrated and precise. The use of anabolic agents can significantly increase overall lean mass synthesis and directly or indirectly improves healing by increasing protein synthesis.

Seating and pressure ulcers: Clinical practice guideline

Abstract
Pressure ulcers (bedsores) often develop following prolonged sitting especially where people are immobile or are unable to feel discomfort due to injury or disease. This guideline provides health professionals with specific recommendations intended to minimise any risk of developing pressure ulcers when their clients are seated. The guideline covers sitting when people are acutely ill in hospital and, more commonly, where people have a long-term need for prolonged sitting for example after spinal injury. Particular focus is placed upon the need to make sure the physical size and shape of the seat is appropriate for the seated individual. Guidance is provided upon areas where seated individuals require information to help them minimise pressure ulcer development. The advantages and disadvantages of different cushion materials are described.

Accuracy of Monofilament Testing to Diagnose Peripheral Neuropathy: A Systematic Review

ABSTRACT:
PURPOSE We wanted to summarize evidence about the diagnostic accuracy of the 5.07/10-g monofilament test in peripheral neuropathy.

METHODS We conducted a systematic review of studies in which the accuracy of the 5.07/10-g monofilament was evaluated to detect peripheral neuropathy of any cause using nerve conduction as reference standard. Methodological quality was assessed using the Quality Assessment of Diagnostic Accuracy Studies (QUADAS) tool.

RESULTS We reviewed 173 titles and abstracts of articles to identify 54 potentially eligible studies, of which 3 were finally selected for data synthesis. All studies were limited to patients with diabetes mellitus and showed limitations according to the QUADAS tool. Sensitivity ranged from 41% to 93% and specificity ranged from 68% to 100%. Because of the heterogenous nature of the studies, a meta-analysis could not be accomplished.

CONCLUSIONS Despite the frequent use of monofilament testing, little can be said about the test accuracy for detecting neuropathy in feet without visible ulcers. Optimal test application and defining a threshold should have priority in evaluating monofilament testing, as this test is advocated in many clinical guidelines. Accordingly, we do not recommend the sole use of monofilament testing to diagnose peripheral neuropathy.
